Appeal of the undersigned health professionals to the Ministers of Health and their representatives meeting on the occasion of the 74th World Health Assembly
An initiative of Doctors4XR Switzerland
Year after year, declaration after declaration, multilateral institutions - including WHO - have warned us: climate change and the loss of biodiversity endanger human health worldwide.
Global warming is an existential threat for future generations. More than a threat, it is in fact already a reality. As frontline health workers, we are confronted every day with the consequences of environmental deterioration on our patients and communities. The list of ailments they suffer from is getting longer every day. We are seeing more and more respiratory and cardio-vascular diseases due to polluted air, loss of working days and deaths due to heat waves, over- and undernutrition due to lack of quality food, and diarrhoea and intoxications due to polluted drinking water.
Despite accumulating evidence, repeated warnings from numerous organizations and successive international conferences, worrisome indicators from monitoring tools and successive assessments, despite more frequent and more serious natural disasters, including the Covid-19 pandemic - probably heralding the emergence of new health disasters -, the concrete actions implemented by our governments are not nearly enough considering the severity and urgency of the situation.
Faced with inertia, inaction and the abyssal distance between speeches and actions, we, the signatories, who witness every day the deterioration of the health of our patients and communities due to climate change and environment degradation, demand:
That health authorities in every country publicly state that climate change is putting people at risk of death, and act now to preserve life.
Health authorities must make the consequences of climate change and biodiversity loss the number one public health issue, and support the declaration of a state of emergency for climate and biodiversity.
The new health goals must be to rapidly restore a safer climate and to ensure the best possible protection of all people and species, especially the most vulnerable.

We ask for a health system that is more respectful of people and the environment.
Here are some examples of necessary immediate actions:
Focus on preventive rather than curative care
- Offer (close-to-)free consultations and preventive medecine initiatives
- Promote healthy food, vegetarian trend in public canteens, climate tax on food provenance, bulk food
- Promote soft mobility, 100% pedestrian zones in city centres, affordable public transportation
Promote a more sober and equitable medicine
- Decrease the use of sophisticated equipment, useless diagnostic tests and unnecessary medication
- Redistribute resources between privileged and disadvantaged regions and people
- Re-humanize medecine by favouring communication over technology
Develop environmentally friendly health structures
- Reduce the carbon impact of hospitals, surgeries, clinics and practices
- Reduce consumables and waste
Focus on community-based care and support
- Create decentralized health care centres close to communities and focused on their needs
- Promote inter-professionalism and task shifting
- Contribute to the care and support of patients and disabled by healthy people (paid activity)
- Concentrate more on vulnerable and precarious populations
Respect the natural environment
- Ensure clean water and clean air (stricter control of industries)
- Restrict/abolish synthetic pesticides, unnatural fertilizers
- Re-vegetate towns
Organize information and education
- Organize preventive medecine courses in schools, universities, associations, populations
Involve the population and patients in strategic decisions
- Create popular/citizen assemblies to decide on strategic orientations, choice of criteria and reimbursable services
- And be accountable to them…

Année après année, déclarations après déclarations, les institutions multilatérales – y compris l'OMS – nous ont averti.e.s: le changement climatique et la perte de la biodiversité mettent en danger la santé humaine dans le monde entier.
Le réchauffement climatique est une menace existentielle pour les générations futures. Plus qu'une menace, il est en fait déjà une réalité. En tant que travailleur.euse.s de la santé de première ligne, nous sommes confronté.e.s chaque jour aux conséquences de la détérioration de l'environnement sur nos patient.e.s et communautés. La liste des affections dont ils et elles sont victimes s'allonge chaque jour. Nous voyons de plus en plus de maladies respiratoires et cardio-vasculaires dues à l'air pollué, de perte de jours de travail et de décès dus aux vagues de chaleur, de malnutrition (pas assez ou trop) due au manque de nourriture de qualité et de diarrhées et intoxications dues à une eau potable polluée.
Pourtant, en dépit des preuves qui s'accumulent, des avertissements répétés des organisations multilatérales, des conférences internationales qui se succèdent, des indicateurs inquiétants des outils de suivi, en dépit des catastrophes naturelles plus fréquentes et plus graves, y compris la pandémie de Covid-19 - probablement annonciatrice de l'émergence de nouvelles catastrophes sanitaires - les actions concrètes mises en œuvre par nos gouvernements ne sont pas à la hauteur de la gravité et de l'urgence de la situation.
Face à cette inertie, à cette inaction, à la distance abyssale entre les discours et les actes, nous, les signataires, témoins quotidiens de la détérioration de la santé de nos patient.e.s et communautés en relation avec le dérèglement climatique et l'environnement, demandons:
Que les autorités sanitaires de chaque pays déclarent publiquement que le changement climatique met les populations en danger de mort, et qu’elles agissent maintenant pour préserver la vie.
Les autorités sanitaires doivent faire des conséquences du changement climatique et de la perte de la biodiversité le problème de santé publique numéro un, et soutenir la déclaration de l’état d’urgence pour le climat et la biodiversité.
Les nouveaux objectifs sanitaires doivent être de restaurer rapidement un climat plus sûr et d’assurer la protection la meilleure possible de toutes les personnes et de toutes les espèces, surtout les plus vulnérables.

Nous demandons un système de santé qui soit plus respectueux de l’humain et de l’environnement.
Voici des exemples d'actions nécessaires immédiatement:
Miser sur le pilier de la prévention plutôt que des soins curatifs
- Offrir des initiatives de prévention et des consultations (presque) gratuites
- Promouvoir une alimentation saine, la tendance végétarienne dans les cantines publiques, une taxe climat sur la provenance des aliments, des aliments en vrac
- Promouvoir la mobilité douce, le 100% piéton au centre-ville, les transports publics à prix abordables
Promouvoir une médecine plus sobre et plus équitable
- Diminuer l’utilisation d’équipements sophistiqués, de tests diagnostiques inutiles et de médicaments superflus
- Redistribuer les ressources entre les régions et les personnes privilégiées et celles qui sont désavantagées
- Ré-humaniser la médecine en favorisant la communication plutôt que la technologie
Développer des structures de santé respectueuses de l’environnement
- Diminuer l’impact carbone des hôpitaux, de la chirurgie, des cliniques et des cabinets de ville
- Diminuer les consommables et les déchets
Privilégier des soins et du soutien de proximité dans la communauté
- Créer des centres de soins décentralisés, à proximité des communautés, centrés sur leurs besoins
- Favoriser l’inter-professionnalité et le transfert de tâches
- Contribuer au soin et au soutien des personnes malades ou handicapées par des personnes en bonne santé (activité rémunérée)
- Cibler plus particulièrement les populations vulnérables et précarisées
Respecter les milieux naturels
- Assurer une eau saine et un air sain (contrôle plus strict des industries)
- Restreindre/abolir les pesticides de synthèse, les engrais non naturels
- Re-végétaliser les villes
Organiser l’information et l’éducation
- Organiser des cours de prévention dans les écoles, universités, associations, populations
Impliquer la population et les patient.e.s dans les décisions stratégiques
- Créer des assemblées populaires/citoyennes pour décider des orientations stratégiques, critères de choix et prestations remboursables

Appell der unterzeichnenden Gesundheitsfachkräfte an die Ländervertreter der 74. Weltgesundheitsversammlung
Eine Initiative von Doctors4XR Schweiz
Jahr für Jahr, durch eine Deklaration nach der anderen, haben uns multilaterale Institutionen - darunter die WHO - gewarnt: 7
Die globale Erwärmung ist eine existenzielle Bedrohung für zukünftige Generationen. In der Tat ist sie nicht mehr nur eine Bedrohung; sie ist bereits Wirklichkeit geworden. Als medizinische Fachkräfte der vordersten Front sind wir jeden Tag mit den Folgen der Umweltzerstörung für unsere Patienten und Gemeinden konfrontiert. Die Liste der durch diese Zerstörungen verursachten Erkrankungen wird jeden Tag länger. In unserer Praxis sehen wir immer mehr Atemwegs- und Herz-Kreislauf-Erkrankungen aufgrund verschmutzter Luft sowie Krankschreibungen und sogar Todesfälle aufgrund von Hitzewellen, Über- und Unterernährung, Mangel an qualitativ hochwertigen Lebensmitteln sowie Durchfall und Vergiftungen durch verschmutztes Trinkwasser.
Trotz überwältigender Beweise, wiederholter Warnungen zahlreicher Organisationen sowie zahlloser internationaler Konferenzen, besorgniserregender objektiver Messungen; trotz häufigerer und schwerwiegenderer Naturkatastrophen, einschließlich der Covid-19-Pandemie - die wahrscheinlich die Entstehung neuer Gesundheitskatastrophen ankündigt, sind die konkreten Maßnahmen unserer Regierungen angesichts der Schwere und Dringlichkeit der Situation, nicht annähernd ausreichend.
Angesichts der Trägheit, Untätigkeit und der abgrundtiefen Diskrepanz zwischen Reden und Handeln fordern wir, die Unterzeichner, die wir jeden Tag die Verschlechterung der Gesundheit unserer Patienten und Gemeinden aufgrund des Klimawandels und der Umweltzerstörung miterleben:
Dass die Gesundheitsbehörden aller Länder öffentlich die Gefahren des Klimawandels für das Leben der Menschen anerkennen und dass jetzt gehandelt werden muss, um Gesundheit und Leben zu schützen.
Die Gesundheitsbehörden müssen die Folgen des Klimawandels und des Verlusts der Biodiversität zum Hauptthema der öffentlichen Gesundheit machen und die Ausrufung des Notstands für Klima und biologische Vielfalt unterstützen.
Neue Gesundheitsziele müssen darin bestehen Ökosysteme wiederherzustellen und rasch den bestmöglichen Schutz aller Menschen und Arten zu gewährleisten, insbesondere der am meisten gefährdeten.

Wir fordern ein Gesundheitssystem, das Menschen und Umwelt respektiert.
Hier sind einige Beispiele für notwendige Sofortmaßnahmen:
Fokus auf präventive statt kurative Versorgung
- Angebot von (nahezu) kostenlosen Beratungen und Präventionsinitiativen
- Förderung einer sanften Mobilität, 100% Fußgängerzone in der Innenstadt, kostenlose (oder bezahlbare) öffentliche Verkehrsmittel, gesunde Ernährung, vegetarischer Trend in öffentlichen Kantinen, Klimasteuer auf Lebensmittelherkunft, nachfüllbare Lebensmittelverpackung
- Förderung gesunder Ernährung, vegetarischer Trend in öffentlichen Kantinen, Klimasteuer auf Lebensmittelherkunft, nachfüllbare Lebensmittelverpackung
Förderung einer gerechteren und Humanmedizin
- Reduzierung des Einsatzes von hochentwickelten Geräten, nutzlosen diagnostischen Tests und unnötigen Medikamenten
- Nüchternheit im Gesundheitssystem und in der Pflege anstreben
- Re-Humanisierung der Medizin durch Ent-Technologisierung
Entwickelung von umweltfreundliche Gesundheitsstrukturen
- Reduzierung der Kohlenstoffbelastung von Krankenhäusern, Operationen, Kliniken und Praxen
- Reduzierung der Verbrauchsmaterialien und der Abfälle
Fokus auf gemeindenahe Versorgung und Betreuung
- Schaffung dezentraler Gesundheitszentren, die nahe an den Gemeinden liegen und sich an deren Bedürfnissen orientieren
- Förderung von Zusammenarbeit und Aufgabenverlagerung zwischen den Berufsgruppen
- Beitrag zur Pflege und Unterstützung von Patienten und Behinderten durch gesunde Menschen (bezahlte Tätigkeit)
- Einbindung der gefährdeten und prekären Bevölkerungsgruppen
Respektieren der natürlich gegebenen Umwelt
- Sicherstellung von sauberem Wasser und sauberer Luft (strengere Kontrolle der Industrie)
- Synthetische Pestizide und unnatürliche Düngemittel einschränken/abschaffen
- Städte wieder begrünen
Information und Aufklärung organisieren
- Präventivmedizinische Kurse in Schulen, Universitäten, Vereinen, in der Bevölkerung organisieren
Einbindung der Bevölkerung und der Patienten in strategische Entscheidungen
- Volks-/Bürgerversammlungen einrichten, um über strategische Ausrichtungen, Auswahlkriterien und erstattungsfähige Leistungen zu entscheiden
- Und diesen gegenüber rechenschaftspflichtig sein...

Un'iniziativa di Doctors4XR, supportata da molti gruppi sanitari e professionisti della salute
Anno dopo anno, dichiarazione dopo dichiarazione, le istituzioni multilaterali - compresa l'OMS - ci hanno messo in guardia su quanto il cambiamento climatico e la perdita di biodiversità mettano in pericolo la salute delle persone in tutto il mondo.
Il riscaldamento globale è una minaccia esistenziale per le generazioni future. Più che una minaccia, è infatti già una realtà. In quanto di operatori sanitari in prima linea, dobbiamo confrontarci ogni giorno con le conseguenze del deterioramento ambientale del clima sui nostri pazienti e sulle loro comunità. L'elenco dei disturbi di cui soffrono si allunga ogni giorno di più. Sul campo, per i nostri pazienti, le cattive notizie si stanno accumulando. Stiamo riscontrando sempre più malattie respiratorie e cardio-vascolari dovute ad aria inquinata, perdita di giornate lavorative e decessi per ondate di caldo, malnutrizione (o per mancanza o per eccesso) di cibo di qualità, diarrea e intossicazioni dovute ad acqua potabile inquinata.
Nonostante l'accumulo di prove, ripetuti avvertimenti da numerose organizzazioni, diverse conferenze internazionali, indicazioni preoccupanti di strumenti di monitoraggio e risultati allarmanti di valutazioni successive, nonostante i disastri naturali più frequenti e più gravi, inclusa la pandemia Covid-19 - che probabilmente preannuncia l'emergere di nuovi disastri sanitari -, le azioni concrete messe in atto dai nostri governi non sono assolutamente sufficienti, considerando la gravità e l'urgenza della situazione.
Di fronte a questa inerzia, questa inazione, la distanza abissale tra discorsi e azioni, i firmatari, testimoni quotidiani del deterioramento della salute dei loro pazienti e delle comunità a causa dei cambiamenti climatici e del degrado ambientale, chiedono:
Che le autorità sanitarie di ogni paese dichiarino pubblicamente che il cambiamento climatico sta mettendo le persone a rischio di morte, e agiscano ora per preservare la vita.
Le autorità sanitarie sono tenute a fare delle conseguenze del cambiamento climatico e della perdita di biodiversità la questione prioritaria della salute pubblica, e a sostenere la dichiarazione di uno stato di emergenza per il clima e la biodiversità.
I nuovi obiettivi di salute devono essere quelli di ripristinare rapidamente un clima più sicuro e di assicurare la migliore protezione possibile di tutte le persone e specie, specialmente quelle più vulnerabili.

Chiediamo un sistema sanitario più rispettoso delle persone e dell'ambiente; ecco alcuni esempi di azioni necessarie e immediate:
Concentrazione sulla prevenzione piuttosto che su una cura a carattere curativa
- Offrire consulenze (prossime) gratuite e iniziative di prevenzione
- Promuovere la mobilità lenta, zone pedonale al 100% nei centri città, trasporti pubblici a prezzi accessibili

Promuovere una medicina più ragionevole ed equa
- Diminuire l'uso di attrezzature sofisticate, test diagnostici inutili e farmaci non necessari.
- Ridistribuire le risorse tra regioni e persone privilegiate e svantaggiate
- Ri-umanizzare la medicina de-tecnologizzandola
Sviluppare strutture sanitarie rispettose dell'ambiente
- Ridurre l'impatto delle emissioni di anidride carbonica di ospedali, ambulatori, cliniche e studi
- Ridurre materiali di consumo e i rifiuti
Promuovere l'assistenza e il sostegno a livello di comunità
- Creare centri di assistenza (prevenzione, sostegno medico-psichico-sociale) decentralizzati vicini alle comunità, focalizzati sui loro bisogni
- Promuovere l'interprofessionalità trasferendo di compiti (medico-infermiere-farmacista)
- Contribuire al sostegno di pazienti e disabili da parte di persone in buona salute (attività retribuita)
- Mirare in particolar modo la popolazione più vulnerabile e precaria
Rispettare l'ambiente naturale
- Garantire acqua pulita e aria pulita (controllo più rigoroso delle industrie)
- Limitare / abolire i pesticidi e i fertilizzanti non naturali
- Ri-vegetalizzare le città
Organizzare l'informazione e l'istruzione
- Organizzare corsi di prevenzione nelle scuole, università, associazioni, popolazione

Ano após ano, declaração após declaração, as instituições multilaterais - incluindo a OMS - têm-nos avisado: as alterações climáticas e a perda de biodiversidade estão a pôr em perigo a saúde humana no mundo inteiro.
O aquecimento global é uma ameaça existencial para as gerações futuras. Mais do que uma ameaça, na verdade já é uma realidade. Como profissionais de saúde na linha da frente, somos diariamente confrontados com as consequências da degradação ambiental sobre os nossos pacientes e comunidades. A lista de enfermidades de que sofrem está a crescer diariamente. Vemos cada vez mais doenças respiratórias e cardiovasculares devido a ar poluído, dias de trabalho perdidos e mortes devido a ondas de calor, desnutrição (demasiado pouco ou demasiado) devido à falta de alimentos de qualidade, e diarreias e envenenamento devido a água potável poluída.
No entanto, apesar da acumulação de provas, repetidos avisos de organizações multilaterais, sucessivas conferências internacionais, indicadores preocupantes dos instrumentos de monitorização, catástrofes naturais mais frequentes e mais graves, incluindo a pandemia de Covid-19 - provavelmente anunciando a emergência de novas catástrofes sanitárias - as acções concretas implementadas pelos nossos governos não são proporcionais à gravidade e urgência da situação.
Face a esta inércia, a esta inacção, à distância abismal entre palavras e actos, nós, os signatários, testemunhas diárias da deterioração da saúde dos nossos pacientes e comunidades em relação às alterações climáticas e ao ambiente, exigimos :
Que as autoridades sanitárias de cada país declaram publicamente que as alterações climáticas estão a colocar as pessoas em risco de morte, e ajam agora para preservar a vida.
As autoridades sanitárias devem fazer das consequências das alterações climáticas e da perda da biodiversidade a questão número um da saúde pública, e apoiar a declaração do estado de emergência para o clima e a biodiversidade.
Os novos objectivos de saúde devem consistir em restaurar rapidamente um clima mais seguro e assegurar a melhor protecção possível de todas as pessoas e espécies, especialmente as mais vulneráveis.

Apelamos a um sistema de saúde que respeite mais as pessoas e o ambiente.
Aqui estão alguns exemplos de acções necessárias imediatamente:
Foco no eixo da prevenção e não nos cuidados curativos
- Oferecer iniciativas de prevenção e consultas (quase) gratuitas

- Redistribuir recursos entre regiões privilegiadas e desfavorecidas e indivíduos
- Re-humanizar a medicina pela promoção da comunicação e não da tecnologia
Desenvolver sistemas de saúde ambientalmente responsáveis
- Reduzir o impacto de carbono dos hospitais, cirurgias, clínicas e consultórios em geral
- Reduzir consumíveis e resíduos
Foco nos cuidados e apoio baseados na comunidade
- Criar centros de cuidados descentralizados, próximos das comunidades, centrados nas suas necessidades
- Promover a inter-profissionalidade e a mudança de tarefas
- Contribuir ao cuidado e apoio de pessoas doentes ou deficientes por pessoas saudáveis (actividade remunerada)
- Foco nas populações vulneráveis e desfavorecidas
Respeitar o meio ambiente natural
- Assegurar água e ar limpos (controlo mais rigoroso das indústrias)
- Restringir/abolir os pesticidas sintéticos, fertilizantes não naturais
- Revegetizar as cidades
Organizar a informação e a educação
- Organizar acções de prevenção nas escolas, universidades, associações, populações
Envolver a população e os pacientes em decisões estratégicas
- Criar assembleias populares/cidadãs para decidir sobre as orientações estratégicas, critérios de selecção e serviços reembolsáveis
